Request Appointment

Patient Forms

Upon entering our office, you will be greeted by one of our friendly staff members.  We will request that you complete our  patient forms which can be located under the "Forms" menu in the New Patient Center. This paperwork provides us with your health history and information on your condition. Although these forms can be somewhat lengthy, they are a vital part in understanding your past health history , current musculoskeletal complaints and overall health.


Consultation

Next, you will have a consultation with one of our practitioners to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for our team to learn more about you, your condition, and  expectations to determine how The Matlock Center can meet your goals.


Examination

After your consultation you will  receive a complete physical examination which may include: reflex testing, range of motion and postural assessment as well as other standard neurological, orthopedic, and physical testing. However, no examinations will be performed without your prior consent.


Imaging Studies

Depending on your specific condition, we may suggest you obtain radiographic imaging (X-ray's) when warranted. These images can help us develop an effective treatment plan and can alert us of  any potential underlying conditions that may require additional testing or a referral to additional specialists. Most of our patients do not require any imaging prior to beginning treatment. The decision to obtain diagnostic images will be made by the clinicians on a case by case basis. If you have any reservations about receiving radiographic imaging  please feel free to discuss your questions or concerns with the doctors. We utilize the imaging services of Open MRI of Phillipsburg as well as the imaging department at St. Luke's Hospital (Warren Campus). 


Report of Findings

Once the information is collected  and examinations are performed, the practitioner will give you a verbal report of all findings and answer any questions including:

  1. What is my preliminary diagnosis?
  2. How can you help me?
  3. How often do I need to come in?
  4. What are some of my other treatment options?
  5. What can I expect from these treatments?

After reviewing your health history, expectations, examination findings, and any potential imaging studies, the practitioner will discuss treatment options that will put you on the best course to achieving your health and wellness goals.


Treatment

 At the conclusion of this initial  appointment, you can choose to  experience your first treatment. This may include Chiropractic Care, Acupuncture or Physical Therapy. We are an interactive clinic, so our patients are free to express concerns or ask questions about the different styles of treatment at anytime.


Wellness Program

Before leaving, our practitioners may encourage you to consider a wellness program to incorporate outside of the office.  If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches.  If you desire,  our wellness team will work with you also to create healthy habits and routines for your lifestyle.  Every person is unique, therefore everyone requires a  customized wellness plan.  The purpose  of our wellness program is for   you  to achieve good spinal alignment, have a  healthy diet, exercise, and maintain a positive mental state.
